Medication reconciliation in surgical patients. A literature review

Abstract

Objective: To identify best practices and scientific evidence available in the medical literature on medication reconciliation in surgical patients. Methods: The literature review took a descriptive documentary approach. A systematic and exhaustive review of the literature was conducted in databases such as Google Scholar, the Regional Portal of the Virtual Health Library, Scielo, PubMed, and Dialnet. Results: Omission and incorrect dosage discrepancies were the most frequent. Prescription errors were reduced through medication reconciliation. The number of unintentional discrepancies was significantly reduced. The existence of unintentional medication discrepancies upon admission of surgical patients can expose patients to potential harm. Conclusions: Reconciliation reduces the number of patients who experience medication errors. Appropriate strategies should be developed to support its effective implementation.
