Orthognathic surgery as a first treatment for severe malocclusion with skeletal discrepancy

Objective: To evaluate the efficacy and long-term stability of the Surgery First Approach (SFOA) in comparison with the Conventional Three-Stage Method (CTM) in the treatment of severe malocclusions with skeletal discrepancy. Methods: A systematic review was conducted, selecting 21 relevant studies from a total of 578 identified. Results: The results show that SFOA can reduce treatment time and improve the patient's esthetic perception. However, less long-term stability was observed compared to CTM. Although SFOA has advantages in reducing discomfort and treatment times, challenges related to skeletal stability and recurrence were also identified. Conclusions: Quality of life assessment indicates immediate improvements in facial aesthetics with SFOA, but also highlights that recurrence may be a major concern with this approach.
