Abstract

Dupilumab controls moderate-to-severe atopic dermatitis, but what happens after it is stopped remains uncertain. Objective: To analyze the evidence on recurrence of atopic dermatitis after discontinuation or dose spacing of dupilumab. Method: Systematized review in PubMed/MEDLINE and SciELO with predefined eligibility criteria; 5 studies were selected. Results: In a randomized trial, only 30.4% of patients switched to placebo maintained an EASI-75 response at 36 weeks, compared with 71.6% of those who continued. In clinical practice, recurrence after discontinuation ranged from 23.4% to 42.8%, with a median of 29 to 47 weeks; dose tapering was successful in 83.3% of patients with low disease activity. Conclusion: Recurrence is frequent but not universal; gradual dose spacing is safer than abrupt discontinuation.
