Abstract

Radicular pathology in older adults generates pain and progressive disability that impairs autonomy. Given the limitations of conservative management, alternatives such as endoscopic spine surgery and pain therapy emerge. Objectives: To compare the clinical efficacy and safety of endoscopic spine surgery versus pain therapy in the management of radicular pathology in older adults. Methods: A comparative, applied, observational, and longitudinal cohort study, with a population of 50 patients equally distributed into two groups (endoscopic surgery and pain therapy), clinically evaluated for 12 months. Results: Endoscopic surgery demonstrated sustained pain relief and lower long-term disability, reducing the need for re-interventions. Conclusions: Endoscopic spine surgery is superior to pain therapy, offering greater clinical efficacy, safety, and functional stability in older adults.
