Abstract

Palpebral xanthelasma is the most common form of cutaneous xanthoma, and its value as a marker of cardiometabolic risk remains under discussion. Objective: To analyze the evidence on the relationship between palpebral xanthelasma, lipid abnormalities, and cardiovascular risk. Method: Systematized review in PubMed/MEDLINE and SciELO with predefined eligibility criteria; 9 studies were selected. Results: A meta-analysis of 15 case-control studies showed higher total cholesterol, LDL cholesterol, and apolipoprotein B, and greater carotid intima-media thickness. In a cohort of 12,745 people, xanthelasma predicted myocardial infarction (HR 1.48) independently of lipids; however, the meta-analysis of events was inconclusive and some studies found no differences. Conclusion: Xanthelasma warrants lipid and global cardiovascular risk assessment, although it is not, by itself, an established predictor of events.
